Job Description:

Shifts: 8-hour days

Department/Unit - Home Health
Occupational Therapy evaluation, reassessments, supervisory visit and home visit. Supervised COTA. Develop and 9implement plan of care to facilitate patient recovery, adult and geriatric population.
Bed Size : 4-5 visits per day depending on type of visits
State license status must be currently active
Minimum of 2 years experience required

Certifications:
BLS

Skills:
Epic remote client is EMR used.
