Job Description:
Position Summary:
The Clinical Pharmacist works collaboratively with the physician
teams, advanced practitioners, medical residents and staff to
promote and improve safe medication use in trauma and neuro
patients. Uses evidence-based medicine to provide recommendations
to optimize drug regimens with a focus of preventing adverse drug
events. Provides patient specific medication use teaching for high
risk patients (i.e. polypharmacy, pediatric patients, patients on
high risk medications or are at risk for readmission due to
medication non-compliance). Assist with drug therapy management in
medical emergencies. Teaches and precepts pharmacy residents,
nursing, and other healthcare providers as assigned.
Qualifications:
EDUCATIONAL REQUIREMENTS
Doctorate Pharmacy or three (3) years of experience with a BS in
Pharmacy is a valid substitution for Doctorate in Pharmacy.
CERTIFICATION & LICENSURE REQUIREMENTS
Pharmacist License Illinois and Basic Life Support (BLS) w/in 30
days of start date in position and Advanced Cardiac Life Support
(ACLS) w/in 1 year of start date in position and Pediatric Advanced
Life Support (PALS) w/in 1 year of start date in position and. BCPS
preferred.
EXPERIENCE REQUIREMENTS
Successful completion of ASHP Accredited Residency program (PGY1)
required. Clinical experience in the area of departmental specialty
or PGY2 preferred. Five years or greater experience in Critical
Care in lieu of residency.
