Job Description:
Position Summary:
The Transfer Center RN Specialist is dedicated to facilitating
appropriate patient transfers into Carle Hospital from referring
physicians and facilities (hospitals, clinics, convenient/urgent
cares, surgery centers, etc.) as well as transfers from Carle to
quaternary medical facilities (outbound transfers), Obtains
appropriate patient information critical to determining the
appropriate Carle provider, medical necessity and level of care,
and demographic information. Responsible for facilitating a
conference call between the referring physician and the Carle
receiving physician, and will ensure, when appropriate, that an
appropriate level of care bed within Carle is identified, and
facilitate a timely mode of transportation. Responsible for meeting
the needs and expectations of referring physicians/facilities, as
well as accepting physicians by supporting and managing the patient
transport process efficiently and effectively. Efficiently engages
appropriate departments in the transfer process and monitors and
documents all activities associated with the transfer. Responsible
for oversight of the entire transfer episode, from start to finish,
for communication back to the referring facility, and for
communication to the physicians assuming care of the patient
post-discharge from Carle.
Qualifications:
EDUCATIONAL REQUIREMENTS
Diploma (Not HS) in Nursing
CERTIFICATION & LICENSURE REQUIREMENTS
Registered Professional Nurse (RN) License Illinois upon hire.
EXPERIENCE REQUIREMENTS
Experience using Epic EMR.
SKILLS AND KNOWLEDGE
- Demonstrated proficiency in computer skills and systems. -
Develops respectful relationships with internal and external
customers by listening, understanding and providing solutions to
their needs. - Demonstrates effective leadership and professional
skills according to Standards of Care. - Demonstrates excellent
professional and clinical skills and utilization of the nursing
process of direct and indirect patient care which includes
assessment, planning, implementation and evaluation of nursing care
to meet patient needs and improve patient outcomes. - Working
knowledge of applicable state and federal regulatory standards and
regulations, such as EMTALA, related to acute and urgent/emergent
patient transports. Knowledge of State, Federal, and Professional
industry standards pertaining to health care practice (e.g. Title
22 and JCAHO). - Exemplary interpersonal communication including
oral, written and over the phone. - Demonstrated team player skills
required. - Requires excellent critical thinking skills, ability to
think quickly and strong organizational skills. - Ability to deal
objectively with a wide variety of situations. Ability to manage
multiple priorities in stressful situations. - Must be able to
establish relationships quickly with key stakeholders / peers
(physicians and others in the health care system), projecting a
friendly helpful image over the phone. - Ability to work
independently and be a self-starter with ability to complete job
duties autonomously. - Work collaboratively on projects with
personnel from a variety of service lines. Able to understand
complex diagnosis and treatment information and use deductive
reasoning. - Requires the ability to resolve conflict effectively
and in the best interest of patients and families. - Requires
strong work ethic and attention to detail.
