Job Description:
Carle Health is looking for a compassionate and knowledgeable RN
Educator, Diabetes to join our Healthcare & Medical Services team.
In this role, you will be responsible for designing, implementing,
and evaluating patient-specific and group educational programs on
diabetes management. By working closely with other healthcare
professionals, you will be integral in enhancing our patient
outcomes through expert diabetes education and care management.
Assess educational needs and learning styles of patients and their
families. Develop and implement educational plans based on
assessments and in compliance with current medical guidelines and
treatments. Conduct group classes and one-on-one sessions to
educate patients on diabetes management, including nutrition,
medications, glucose monitoring, and lifestyle adjustments.
Collaborate with other healthcare providers to tailor interventions
and monitor patient progress. Maintain patient documentation and
education records in accordance with HIPAA laws and institutional
policies. Stay current with advancements in diabetes care and
education techniques through continuous professional development.
Strong interpersonal and communication skills In-depth knowledge of
diabetes pathology and treatment Ability to assess patient needs
and develop effective educational programs Proficiency in
electronic health record (EHR) systems Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current Registered
Nurse (RN) license in the state of practice. Certified Diabetes
Educator (CDE) or willing to obtain certification within one year
of hire.
